Method and decision boundary
The coverage score counts seven required event elements—asset, time, symptom, phase, duration, operating state, and recovery—plus the six evidence controls shown above. It measures completion of this declared capture form, not evidence accuracy or fault certainty.
Preserve before changing
Export logs and preserve the as-found state before resets, connector movement, configuration changes, or part swaps when safety and approved procedures allow. A disappearing symptom is part of the evidence; it is not proof that the fault is cleared.
Limitations
This report does not diagnose a cause, approve troubleshooting actions, or release equipment. Follow approved manuals, hazards and energy controls, data-retention requirements, and qualified engineering review.
